Your Guardian PPO dental network is DentalGuard Preferred. Note that all plan provisions, including deductibles and copays are the same for 2022 as they were for 2021.

The dental PPO plan, works the same way as the medical PPO. Your share of the cost is determined by whether care is received by a participating or non-participating provider.

If a participating provider is used, there are no deductibles and you pay a pre-determined percentage of negotiated rates. You can expect to pay more than the coinsurance amount if you use a non-participating provider. Non-participating providers may bill you for any amount up to the billed charge after the plan has paid its portion (a percentage of usual and customary) of the bill.
